Many people know that St. Patrick’s Residence Nursing and Rehabilitation is a long-term care facility for the elderly. It is a special place where the needs and well-being of our residents are addressed with care and lots of love. But St. Patrick’s also offers short-term rehabilitation to individuals who have a need for this service due to hip/knee replacement surgery, back surgery, stroke or neurological issues requiring attention on a short-term basis.

Offering a “Transition Back Home”...

The goal of short-term rehabilitation is for the resident to return to their prior level of functioning in their own home, assisted living facility or with their families. As their care begins, our rehabilitation team looks at the resident in a holistic manner. This team includes physical/occupational/speech therapists, social services, dietary, nursing, pastoral care and activities. We also start the plan for discharge at admission by learning what the resident must be able to do to go back home and our therapists will perform an in-home evaluation and design an individualized program to be continued at home. St. Patrick’s wants to alleviate the stress of going back home to a normal routine and wants to make sure the individual and their families are comfortable with the home program.

The resident works with our trained, certified therapists on physical, occupational and speech therapies. The length of sessions varies depending on the amount of therapy needed and the residents’ stamina. Therapy sessions are given five days a week and weekend sessions are available based on need. Families are welcome to observe sessions if they wish and can call us anytime. Daily Mass, Rosary and all activities are also available to our short-term residents.

The Sisters Are Always Near...

The Carmelite Sisters are a comfort to those individuals who will be leaving their homes, although for a short time, to face the rigors of therapy. Our philosophy of care is based on the Mission set before us by Venerable Mary Angeline Teresa, Carmelite Foundress, of treating the elderly with respect and dignity.  It is the difference that sets us apart from other facilities, but the real “Difference is Love”sm.
